As a Care Coordinator, your duties involve evaluating patients' health, guiding them on adopting healthier lifestyles like proper nutrition and exercise, and equipping them with tools to achieve specific goals in a designated care plan. In this role, you will work autonomously, overseeing a monthly patient caseload to ensure all individuals receive necessary care. To excel as a care coordinator, you should display a positive attitude, strong clinical abilities – especially in managing hypertension and obesity, effective communication skills, and a dedication to excellence in patient care and customer service. Care Coordinator Responsibilities: <ul><li>Collaborate with Nurse Practitioners to develop care plans for patients</li><li>Regularly follow up with patients to assess blood pressure or weight readings and adherence to care plans. Offer guidance to address unhealthy behaviors</li><li>Evaluate critical findings and liaise with Primary Care Provider (PCP) to help them implement a treatment plan for the patient</li><li>Record all interactions and details in Electronic Health Record (EHR) chart notes</li><li>Manage and organize a high volume of patient calls efficiently</li><li>Schedule: 9am-5:30pm Eastern Standard Time </li></ul>Care Coordinator Requirements: <ul><li>Preferably holds an LPN or RN degree with an active, unrestricted license (in a state that is part of the Nurse Licensure Compact)</li><li>Possesses excellent clinical knowledge concerning the aging adult population and healthy practices</li><li>Demonstrates strong organizational skills and attention to detail</li><li>Proficient in using various systems and software programs</li><li>Previous experience in health coaching, fitness, or nutrition is advantageous</li><li>Capable of working full-time from a remote or home-based location</li><li>Maintains a pleasant phone manner and shows compassion</li><li>Self-managing in a fast-paced, high-volume work environment with a willingness to assist in other projects during downtime</li><li>Availability during standard business hours, 9am-5:30pm Eastern Standard Time with flexibility across national time zones as needed</li><li>May involve occasional shared on-call responsibilities and after-hours coverage </li></ul>Life Line Screening is proud to be an equal opportunity employer.
