As a Licensed Master Social Worker at Central New York Psychiatric Center, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness.. Duties Description As a Licensed Master Social Worker at Central New York Psychiatric Center, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness. Duties include: <ul><li> Providing crisis intervention, service linkages, and discharge planning</li><li> Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed</li><li> Determining discharge needs and appropriate community resources</li><li> Establishing and maintaining relationships with community providers to ensure continuity of care </li></ul>NOTE: <ul><li> In order to be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and/or State Medicaid and Medicare exclusion lists (or excluded from any other Federal or Federally assisted program).
