- Behavioral Health Care Provider (BCP), organizes and implements the prescribed Behavioral Health Integrated Model of Care and clinical Psychotherapy for NVHC patients and their families; serves as a therapist and resource person for NVHC patients in accordance with the medical and behavioral plan of treatment; abides by the National Association of Social Work Code of Ethics for social work in a healthcare setting.
- Provides behavioral health services including behavioral health screening; psychosocial assessment; SBIRT; DSM V and ICD9/10 diagnosing.
- Provides individual, family, and group psychotherapy for adults, adolescents and children from diverse backgrounds; crisis intervention; treatment, discharge and aftercare planning.
- Develops and implements individualized treatment plans with goals that are problem specific, measurable, achievable, time limited, and culturally sensitive, to assist patients and families to cope and/or restore social, emotional, financial and environmental factors that affect and/or are affected by illness.
- Collaborates and consults with NVHC Primary Care Providers (PCP) and staff about the behavioral health and psychosocial needs of patients and the impact of these needs on the patient’s health care and compliance with treatment.
