- The lead case manager is responsible for providing housing leadership and guidance to all other case managers on the site and assisting the program manager with special projects.
- The lead case manager will give participants one-on-one mentorship, case management services, and assistance and referrals to needed services.
- The lead case manager is also responsible for assigning clients to case managers and overseeing the input of data into HMIS (Montgomery County Housing database) and ETO (LAYC database) to ensure client assessments and profiles are completed and up to date.
- The lead case manager is also responsible for ensuring the Individual Development Plan (IDP) for all clients is updated and completed.
- Provide crisis intervention and coordinate response with the MoCo Reconnect Mental Health Counselor(s) when needed.
