- This job requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric.
- This job provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services.
- Negotiates and collaborates with the interdisciplinary team, community resources and payers to facilitate timely and cost effective discharge plans that enhance appropriate health and social services in the continuum of care.
