- PROGRAM/DEPARTMENT DESCRIPTIONAs a member of the Division of Education and Community Engagement, the Coordinator III, in conjunction with the Senior Training Program Manager will develop and implement educational initiatives, multi-session training institutes as well as shorter workshops with BPHC colleagues and partners on topics related to HIV, infectious disease prevention, health equity and racial justice.
- The Case Management Training Program is a Ryan White Part A funded program created to provide training to providers funded to deliver Medical Case Management (MCM) and non-Medical Case Management services to People Living with HIV (PLWH) within the Ryan White Boston Eligible Metropolitan Area. The PCIII will partner organizations to provide training to case managers, to ensure PLWH can reach and maintain viral suppression by accessing the full continuum of HIV services.
- This includes conducting training on needs assessment, individualized service plans, developing racial justice and equitable practice training for individuals living with HIV (PLWH/A), ensuring proper access to behavioral health services, and other relevant case management training.
