- The Community Health Nurse (CHN) is a highly skilled nurse who provides quality primary health care service to special populations to promote and preserve the health of individuals, families and groups to improve the health of the community.
- Staffs daily RN triage appointments to support acute care needs and chronic disease management education, treatments, and assessment of participants across the lifespan of participants.
- Monitors the RN triage phone line to address participant care needs including: medication refills, lab results, triaging acute symptoms, and care coordination requests as appropriate from participants and/or hospitals clinics, pharmacies and other external health partners.
- Provide injections for special populations including: PrEP, HIV management, STI treatment, hormone replacement therapy, antipsychotic long acting injectable, Medication Assisted treatments, and family planning.
