- The position of Service Coordinator for Child & Family Connections #19 is a full-time position providing service coordination/case management for children and their families in the CFC #19 region (Clark, Coles, Cumberland, DeWitt, Douglas, Edgar, Macon, Moultrie, Piatt and Shelby Counties).
- The Service Coordinator facilitates the progress of children and their families through intake, evaluation, eligibility determination, the Individual Family Service Plan (IFSP) process, and review of progress as part of the Early Intervention (EI) service system.
- The Service Coordinator maintains a case load and conducts home visits to meet with families.
- The individual must have reliable transportation, as Service Coordinators use their personal vehicle for travel and are reimbursed for mileage at a rate equivalent to the IRS mileage rate.
