- JOB SUMMARY:The Case Manager provides a range of comprehensive community-based case management services, including housing services and trauma-informed case management for individuals who are homeless and/or returning to the community from incarceration.
- The position serves as a liaison to the District of Columbia Department of Corrections, READY Center, Anchor Mental Health (Core Service Agency), and Housing, to identify and coordinate community resources and make the appropriate referrals.
- Interview and assess the needs for direct services of Returning Citizens (post-release) and individuals referred by jails or prisons including the DC Jail prior to discharge (pre-release), primarily in the District of Columbia, who have mental health needs and/or a history of addictive behaviors.
