- Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts.
- Job Summary: The Case Manager performs as an essential part of a multidisciplinary team of medical professionals, clinicians, and Recovery Specialists to provide support, counseling, and community referrals to individuals with substance use and co-occurring disorder, including facilitating individual and group counseling sessions and providing aftercare case management planning services.
- Under the guidance of the Clinical Director, engage patients in information gathering and decision making to inform the development of the psychosocial assessment, the setting of treatment goals, determination of service needs, and aftercare planning.
- Coordinate services with external treatment providers, outpatient counselors/therapists, opioid treatment, and other partners such as primary care, DCF, Courts, and assist patients to coordinate and attend essential appointments as needed during their stay.
- Provide individual and group education and recovery support appropriate to patient medical and mental status during detox/withdrawal and within the limitation of assessed competency and qualifications.
