- The Care Coordinator directs and organizes wound care services in collaboration with the Medical Director and Vice President of Operations for Restore First Health in accordance with applicable federal, state, regulatory, and company standards to ensure the delivery of quality patient care is rendered at all times, and that the Plan of Care and physician orders are followed with regard to established clinical protocols.
- The Care Coordinator plays an important role in the implementation and compliance of Restore First Health's Mobile Advanced Wound Therapy Program's Policies and Procedures, in conjunction with the national standardization process, to include patient care protocols, documentation tools, wound care formulary and referral criteria.
- As a Care Coordinator of the Advanced Wound Therapy Program, this position directs clinical team members on planning and scheduling of wound care visits to include wound team travel route planning, medical records, ordering and inventorying amniotic allografts, tracking and logging of tissue IDs, and close coordination with wound care patients, their referring physicians, and various healthcare entities.
- Nursing Degree from an accredited college or university, or is a graduate of an approved RN or LPN program.
- The Wound Care Coordinator is responsible for working closely with our Authorization Specialist to confirm coverage benefits and prior authorizations are obtained prior to services being rendered.
