You will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, and ensuring quality assurance.
- Responsibilities and Duties:Provides coverage for call-out, new clients, sick, vacation, and long-term leave.
- When a change of caregiver is done, do a case acceptance form.
- Communicate between caregivers, client, case manager, and regarding changes to client health, schedule, and caregiver.
- Hospitalization; report to everyone in the client's care team and fill out a form.
