- In addition, we provide to families case management services, advocacy, referrals, assistance with childcare, housing, income support, food pantries, and medical and mental health services.
- Case Planner will have a caseload of 12 families.
- Case Planners will perform assessments of a child or children and family needs; implementing preventive services to avert, when possible, foster care placement and disruption of the family unit.
- Conducts case work contact visits 2X per month; in the home; in the community; and/or at school.
- Provides and arranges for appropriate referrals; community based, and other services related to the needs (i.e., social, and emotional, therapeutic services, medical, educational, vocational, financial, legal, childcare, home management, entitlement advocacy, substance abuse treatment, anger management, and parenting skill training) for the child/ren and family.
