- The Integrated Mental Health Therapist-BHN is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients.
- The incumbent uses a variety of theoretical treatment models and interventions and excellent communication skills to work within a primary care/chronic disease management team setting with limited supervision; documents treatment/communication in keeping with department standards and keeps current on community resources to effectively provide linkage and advocacy to fulfill client’s needs.
- Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field
- Current licensure as a Licensed Clinical Social Worker Associate (LSWAIC), Licensed Advanced Social Worker Associate (LAASW), Licensed Mental Health Counselor Associate (LMHCA), or Licensed Marriage and Family Therapist Associate (LMFTA) or eligibility for associate level licensure upon hire
