Reporting to Forefront Living’s Chief Operating Officer (COO) and working with the CEO, CFO and COO as a member of the leadership team, the Corporate Director of Home & Community Based Services (HCBS) of Faith Presbyterian Hospice will lead a renowned and well-respected hospice program and HCBS services that include the T. Boone Pickens Center in Dallas, Texas.

Assures that services are provided by Community-Based Specialist(s) under the Homebuilders Model with fidelity to all families and youth referred by the Departments of Social Services or other referral sources.

The Director of Community-Based Services oversees all administrative functions of the Community-Based Program using the Homebuilders Model. The Director of Community-Based Services will be required to work closely with the HOMEBUILDERS® consultant as required by model trainers and cooperate with record reviews and onsite visits.
