Intake Coordinator ORGANIZATIONAL RELATIONSHIP: Reports to DCS RISK OFOCCUPATIONAL EXPOSURE TO BLOOD BORNE PATHOGENS: C: no exposure ?QUALIFICATIONS High School graduate. A minimum of one (1) year workingexperience, prefer in health care setting. Excellent communicationSkills. Must Have Criminal Background Check. RESPONSIBILITIES/ESSENTIALFUNCTIONS Intake & Enrollment Receive, respond and process all referralsfrom individuals, families, case managers, care coordinators, digitalmarketing team and state agencies Conduct initial intake screenings todetermine service interest, eligibility status, and next steps Collect,review, and track required intake documentation, includingauthorizations, releases of information, service referrals, anddemographic information Verify member eligibility and enrollment status,as applicable Coordinate outreach activities with Program Coordinators,nursing staff, and leadership to ensure timely service start datesMaintain accurate and organized intake records in agency systems andparticipant files Communicate intake status updates to individuals,families, and referral sources Outreach & Community Engagement Developand maintain positive relationships with case management agencies,hospitals, community providers, and state partners Conduct outreachactivities to promote A Caring Heart Nursing Services, LLC programs andservices Respond to inquiries from the community regarding availableservices and eligibility pathways Support marketing and outreachinitiatives in coordination with agency leadership Track outreachcontacts, referrals, and follow-up activities Compliance & DocumentationParticipate in QA process of the agency Ensure intake practices complywith program regulations and applicable Medicaid HCBS waiverrequirements Intellectual Property rights of the trademark for A CaringHeart Nursing Services Maintain confidentiality of all participantinformation in accordance with HIPAA and state privacy requirementsAssist with audit readiness by maintaining complete and accurate intakedocumentation Follow agency policies related to intake, enrollment, andrecordkeeping SPECIAL EQUIPMENT TO BE OPERATED Office equipment- phone,computer, printer, copy machine WORK ENVIRONMENT Indoors, performingcomputer/office duties- phone work Occasional travel toreferrals/meetings; will require transportation to get to various sitesFUNCTIONAL ABILITIES Able To Hear, Speak, Understand And Communicateeffectively in English. PHYSICAL REQUIREMENTS On a daily basis: walking,standing, sitting, reaching, use of telephone, use of computers & otheroffice equipment, climbing stairs.
