Description$10,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment.The Transition Coordinator's primary responsibility is to deliver the most remarkable patient experience, in every dimension, every time and understand how he or she contributes to the health system's vision of achieving that commitment to patients and families. The Transition Coordinator is an experienced nurse who is able to provide 1:1 instruction and education to patients/families who will and have experience a surgical procedure in their area of focus (ex. cardiothoracic, orthopedic). The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
