Apply today to our Manager of Social Services and be a part of the innovation of ideas.Job Summary and QualificationsAs Social Services manager has primary oversight of the ED Social Work supervisor and staff in their job duties as they provide direct care to a defined population in the Emergency Department. Oversees the provision of clinical assessment, intervention, social, emotional, and environmental care provided to ED patients by Social Work. Participates in providing patient specific care standards as directed and follows service excellence standards to ensure high levels of patient satisfaction. Provides oversight to the planning and implementation of social work policies and procedures and maintenance of on-call staff schedule. In this dual duty role, as the High Alert Program Manager (HAP), the manager functions as a resource and expert consultant to the high alert team and medical staff for St David's HealthCare on managing care plans for patients with complex psycho-social needs. In addition, the HAP Manager coordinates the HAP software which includes training clinical staff, granting and managing access to the HAP software, maintaining credibility of HIPPA, managing patient data for care plan review, and coordinate with multiple committees and appropriate boards. You will provide supervisory assistance for social servicesYou will hire, train and conduct orientation of Full-time and PRN staff.You will provide supervision and evaluation of Social Work Full-time and PRN staff.You will coordinate monthly on-call schedule for coverage of afterhours Social Services. Facilitate monthly PRN meetings, trainings, completion of required meetings/in services. Provide 24/7 on call support to staff to help address questions, staff cases and process appropriate dispositions.You will coordinate supervisory requirements of LCSW applicants and graduate social work student as needed. Conduct weekly supervision meetings. Facilitate the selection of graduate interns and internship orientation/training needs. As a TSBSWE approved clinical supervisor, provides necessary paperwork to TSBSWE and supervisee.You will assist and advise the executive and leadership team with issues impacting hospital social work.You will regularly consult HR Generalist to address hiring and termination questions, issues pertaining to the Standards of Conduct and Employee Discipline and other matters as needed.You will facilitate services and trainings within the department as needed to adequately and thoroughly address new resources/services or process/ procedures.You will perform complex discharge planning facilitation for designated patient PopulationYou will provide patient/family counseling, discharge planning, community resource/long range planning and crisis intervention by referral You will evaluate psychosocial needs of patients/familiesYou will coordinate reporting and standards of practice review for social work services.You will review and revise social work policies and procedures in accordance with hospital and regulatory agency standards annually and as indicatedYou will monitor and coordinate CQI/QA reporting to director and other hospital committees as required.You will serve as St. David's Healthcare community liaison on matters concerning the high alert Create and manage working data base of clinical care plans for patients identified as meeting criteria for High Alert Program.You will collaborate and consult with Medical Directors to create and manage chronic pain letters.You will consult with patient care providers and unit directors to provide education and act as resource.You will collaborate with legal staff and clinical staff regarding patient care plans.You will oversee and direct training clinical staff on how to utilize HAP software and maintain confidentiality of all assigned network activities related to the HAP.You will manage and maintain regulatory and compliance of the HAP software including HIPPA and IT maintenance and all pertaining HCA guidelines.What qualifications you will need:Masters Degree1 - 3 years: staff nurse in an acute care settingMinimum 5 years of active social work experience, including 3 years in a health care facility.Masters Degree- Social WorkCurrent Licensure by the Texas state board at the LMSW level Required Upon Hire (for certifications only: upon hire is defined as within 2 business days of start date)St. David's Medical Center, part of St. David's HealthCare, is a comprehensive medical facility with three locations: St. David's Medical Center, Heart Hospital of Austin, and St. David's Georgetown Hospital.
