Hours/Shifts
Work schedules are based on agency needs and may include working evenings, weekends, with a variety of shifts and hours.
Description of Duties and Responsibilities:
Responsible for assisting 40+ clinicians with targeted case management of current caseload.
Works in conjunction with the Clinical Director(s) on all targeted case management related activities.
Assess resource needs of clients and assist in creation of Targeted Case Management Care Plan in a timely manner.
Educate and refer clients to community agencies & businesses that appropriately address the clients’ needs for food, shelter, clothing, employment, transportation, etc., and assist in developing community resource agency list with real contacts and phone numbers.
Assist clients with establishing or re-establishing support systems within their community.
Act as an advocate on behalf of the client to improve access to needed services.
Participate in clinical staffing decisions regarding the patients’ current strengths and resources & their need for additional resources.
Responsible for inputting information in HMIS/CMIS (Homeless Management Information System/Community Management Information System).
Assist in other duties as assigned.
Qualifications:
Knowledge and experience with case management.
Ability to work well in a team environment.
Ability to operate a computer and complete documents using electronic health records.
Excellent verbal and written communication skills.
Bachelor’s Degree in a health-related field.
Ability to obtain an NPI and enroll with Nevada Medicaid
