In 2023, we rebranded as NovumHealth with the expansion of our services in Arizona, California, and Utah.Position: Case Manager - Homeless Resource CenterSchedule: Monday - FridayHours: 7:30 am - 4:00 pm.Location: Las Vegas, Nevada – Onsite (This role is not eligible for Remote or Hybrid)Starting Salary Range: $21 per hourThe starting salary range for this role is a reasonable estimate, with a goal to pay a competitive market salary focusing near the median of our pay ranges. However, final offers for all positions will be based on several factors such as experience level, education and certification, skills, and internal pay equity.What’s the role?The Case Manager is responsible for providing comprehensive case management and care coordination services to individuals and families, advocating for their needs, and coordinating resources to ensure optimal member outcomes. This role focuses on addressing social, clinical, emotional, and practical factors that may impact an individual's well-being and ability to access necessary services.What you’ll do: Track patient cases after they are discharged from acute care settings to outpatientbased care by proactively communicating with patients to check on medication and appointment adherence and risk of readmission Assist with hospital census and discharge monitoring and associated reporting, including phone calls to network hospitals to check on current census information Assist with records maintenance related to Novum’s network groups, contracts and communications Assist Practice Manager and clinical leaders as needed for member services including grievance handling, scheduling of urgent appointments, and patient checkin Assist with daily voicemail inbox patient inquiry handling from call center Develop individualized case management plans in collaboration with members, identifying goals, interventions, and necessary resources. Coordinate and connect clients with appropriate community resources, such as healthcare services, housing assistance, financial support, counseling, and educational programs. Provide ongoing support, advocacy, and care coordination to clients and their families, addressing barriers to accessing services and promoting selfempowerment. Monitor and evaluate patient progress towards established care plan goals, adjusting the case management plan as needed and documenting interventions and outcomes. Assist clients in navigating complex systems, such as healthcare, social services, and legal processes, providing guidance and support. Document all case management activities, assessments, and interventions accurately and in a timely manner, adhering to privacy and confidentiality regulations. Excellent verbal and written communication skills, attention to detail in documentation of communication with patients and families Excellent interpersonal skills to establish rapport with clients and effectively collaborate with team members Crisis intervention skills and the ability to provide emotional support and advocacy to clients in challenging situations. Cultural competence and the ability to work with diverse populations, respecting individual differences and promoting inclusivity. Strong documentation and recordkeeping skills to maintain accurate and confidential case records. Ability to navigate complex systems and effectively advocate for clients' rights and access to services. Critical thinking and problemsolving abilities to address barriers and find creative solutions for clients' needs. Commitment to ongoing professional development and ethical standardsEducation and Experience:Minimum of 2 years of experience in social work, care coordination, case management, or a related field, preferably in a healthcare or community-based setting, experience in psychiatric care or therapy also preferred.Education: Bachelor’s Degree in psychology, social work, healthcare or a relevant field required. License, Certificate Or Registration A licensed therapist or social worker is desired but will consider education and experience.
