Duties include telephonic assessment, care planning, implementation, coordination, monitoring, evaluation and outcomes measurement within the scope of their license. The LPN care coordinator is a patient advocate that promotes safety and maintains privacy and confidentiality in accordance with policies and procedures.This position is full-time, Monday - Friday, no weekends or nights!
