From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.What You Will DoCoordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and familiesThis is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). What You Will NeedValid NYS LPN LicenseMinimum 1 year LPN experiencePrior hospice/ home care experience preferred, but not requiredStrong computer skillsOrganization and communication skills and ability to work as part of a teamTraining provided in end of life care.Community Hospice provides a thorough orientation, preceptors and a supportive work environment. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
