The Community Hospice has a wonderful opportunity for a Per diem RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes.Needs are on Weekdays (8a-4:30p) as needed. From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.What You Will DoYou will case manage patients in the Nursing home setting and respond to symptom management needs of our Hospice patients and coordinate with our team to provide added support to residents and families. Work with interdisciplinary team. What You Will NeedValid NYS RN license required.Minimum 1 year RN experience required. 1-2 years long term care RN experience preferred.Previous Hospice experience preferred.Ability to learn/use an electronic medical recordClean Driver's License and reliable vehicleStrong interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkersCommunity Hospice provides a thorough orientation, preceptors and a supportive work environment.Apply today for more information. We can also arrange a time for you to shadow one of our preceptors.Special Per Diem Rates based on Monthly Commitment**Pay Range for non-per diem full-time or part-time Hospice RN employment: $38.00-$50.18 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
