Remote Registered Nurse

Title: RN – Case ManagerLocation: Remote (Candidate must reside in Michigan)Duration: 12 monthsPay Range: $38.00 - $40.00Shift: 8am-5pm EST; Monday - FridaySummaryThe Case Manager RN is responsible for coordinating comprehensive, patient-centered care for members with diverse medical and social needs. Acting as a single point of contact, they collaborate with multidisciplinary teams and use digital and telephonic communication to ensure effective care management.Key Responsibilities:Lead and coordinate multidisciplinary teams (including social workers, dietitians, pharmacists, and medical directors).Assess members’ medical, psychosocial, and support needs.Develop, implement, monitor, and adjust individualized care plans.Communicate with members, caregivers, and providers via phone, email, and digital platforms.Arrange and coordinate resources such as community services, mental health support, and transportation.Educate members on health management, medications, and lifestyle practices.Facilitate care transitions (e.g., hospital discharge to home care).Advocate for members and support self-care and independence.Maintain accurate documentation and meet quality and productivity standards.Requirements:RN with active Michigan state license5 years clinical experience (case management, Emergency Room, ICU, etc.)Experience with telephonic and digital care management is a plus.Strong clinical knowledge and communication skillsManaged care/insurance experience preferredProficiency in Microsoft Office toolsNice to Have:Case Management certificationICONMA is an Equal Opportunity Employer.
