ApplyJob TypeFull-timeDescriptionKlarus Home Health is seeking a dedicated and skilled Personal Care Community Liaison for their Denver, CO personal care office.The Personal Care Community Liaison is responsible for ensuring high-quality personal care services in the field while supporting community outreach and referral development. This role partners closely with caregivers, clients, case managers, and community organizations to deliver exceptional care, maintain compliance, and strengthen community relationships.The ideal candidate is organized, compassionate, professional, and comfortable balancing field supervision with relationship-building and outreach activities.RequirementsConduct client supervisory visits, reassessments, and in-home quality assurance visitsMonitor caregiver performance, including professionalism, punctuality, and documentation complianceProvide caregiver coaching, training, and performance support as neededEnsure care plans are followed, and client needs are metIdentify and report changes in client condition, safety concerns, or service issuesAssist with investigations related to complaints, incidents, or missed visitsMaintain compliance with state regulations, agency policies, and EVV requirementsParticipate in on-call or after-hours support as assignedCare Coordination & Team SupportCollaborate with scheduling, intake, leadership, and caregivers to address client needsAssist with caregiver introductions, client transitions, and new service startsSupport client and caregiver retention through proactive follow-upEnsure timely completion of required documentation and supervisory tasksMaintain accurate and up-to-date records within agency systemsCommunity Outreach & MarketingBuild and maintain relationships with referral sources, including healthcare providers, assisted living facilities, hospitals, and case managersConduct regular outreach and follow-up with community partnersRepresent the organization at community events, meetings, and networking opportunitiesSupport marketing initiatives to drive brand awareness and referral growthTrack outreach activities and provide updates to leadershipQualificationsExperience in home care, healthcare, or personal care services requiredSupervisory or leadership experience preferredStrong communication, organizational, and customer service skillsAbility to work independently and manage multiple prioritiesKnowledge of Medicaid, personal care services, EVV, and state regulations preferredProficiency with Microsoft Office, email, and electronic documentation systemsValid driver’s license, reliable transportation, and proof of insurance requiredPreferred AttributesPositive, professional, and compassionate demeanorStrong relationship-building and communication skillsClient-focused mindset with a commitment to quality careProblem-solving ability and adaptabilityTeam-oriented with a high level of accountabilityPhysical RequirementsAbility to travel locally for field visits and community outreachAbility to sit, stand, bend, and lift up to 25 pounds as neededAbility to safely enter client homes and community settingsBenefits For Full Time EmployeesMedical / Dental / Vision InsuranceShort / Long Term Disability InsuranceAccident, Critical Illness, Cancer Insurance401k Plan w/discretionary company matchCompany Paid Life Insurance80 hours of PTO after 60 days6 Paid Holidays & 1 Floating HolidayMileage reimbursementCompany-paid life insuranceCompetitive compensation
