This full-time role involves working 7 AM to 7 PM shifts, three times a week, including every other weekend and rotating holidays.Essential Responsibilities:Perform duties within the scope of practice defined by certifying boards. Demonstrate skills and knowledge to care for patients from newborn to geriatric stages, following individualized care plans. Monitor and report physical, emotional, and psychosocial patient needs, including symptom and pain management, to the hospice team promptly. Maintain accurate and timely documentation in compliance with privacy regulations. Foster effective communication and professional relationships with patients, families, healthcare team members, and external agencies.Education and Experience:Successful completion of an approved Home Health Aide program.Licenses and Certifications:Current Home Health Aide certification required; CNA certification preferred. BLS/CPR certification must be obtained within 30 days of hire or by orientation end. Valid driver's license and auto insurance required if driving is part of the role.Preferred Qualifications:Previous experience as CNA or HHA, especially in end-of-life care.Skills and Abilities:Basic computer proficiency, multitasking, prioritization, and effective communication. Ability to apply patient care principles with empathy and professionalism.Physical Requirements:Ability to perform heavy work occasionally lifting up to 100 lbs and frequently lifting up to 20 lbs. Standing or walking 51-75% of the workday, with or without accommodation.
