ACT Team Case Manager | Certified Mental Health Specialist Bushton, KS $46,009 - $58,099 a year Full Time Join a dedicated Assertive Community Treatment (ACT) Team that supports individuals with severe and persistent mental illness who have not found success with traditional clinic-based services. This role involves outreach, enrollment, and providing comprehensive mental health and social service support to help clients secure and maintain housing and work towards a fulfilling life within their community. Working closely with clinicians, medical providers, and support staff, you will deliver services with increased intensity and frequency to ensure clients remain supported in their environments. Exceptional customer service and respectful relationship-building are key components of this position. Essential Functions Provide case management using evidence-based, trauma-focused, and person-centered practices to improve emotional and functional well-being. Collaborate with the ACT Team to develop crisis prevention plans and individualized treatment goals. Coordinate care with community partners, caregivers, family members, physicians, and other providers. Support clients in achieving treatment goals, medication education, crisis management, and daily living activities. Assist clients in attending mental health, medical, and social appointments. Complete discharge planning and facilitate transitions to community or higher levels of care. Maintain timely and accurate documentation as required. Participate in a program culture focused on person-centered, outcome-oriented continuous quality improvement. Work a flexible schedule including some evenings, one weekend day, and on-call coverage as needed. Education and Experience Bachelor's degree or equivalent experience in a related field is required. Preferred degrees include Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics. Three to five years of professional experience in mental health services, crisis response, or related fields is preferred. At least two years of experience educating and teaching clients about self-care and improving daily living activities is advantageous. Licensure and Requirements Completion of CPST Case Management online training, KHS online training, and passing background checks including KBI, child abuse, adult abuse registry, and motor vehicle screening is required. Adult Case Management training must be completed within 60 days of employment. Valid driver's license, insurance, and reliable vehicle are necessary.
