LPN Nurse for Supervisory Visits - Home Care - Dublin , GA

We are looking for a motivated and compassionate LPN Nurse to join our team. In this role, you will be responsible for admitting and monitoring clients (patients) in their homes, and also providing supervision, feedback and guidance to nursing assistants working in the home. You will assess patient needs, develop and implement nursing care plans, and monitor patient progress. You will also be responsible for communicating with other members of the care team and providing education to patients and their families. You will submit timely written (or electronic) home visit reports to your nursing supervisor every week.Our ideal candidate is a licensed LPN with a minimum of one year of experience in a home health care setting. For this position, we ask that you commit to visiting 5 to 12+ clients every two (2) weeks and have the ability to travel within a 50 mile radius. A valid driver's license and reliable transportation is required.DUTIESA) Works to promote maintain and restore healthy living in the community for homecare clients and confer with your nursing supervisor RN regarding changes in the Plan of Care.B) Instructs and supervise through the educating of the home health aide, clients and family members, the prevention of infection, accident, and injury.C) Completes an initial Admission Assessment and on-going supervisory visits to monitor home care services.D) Assessment of the client's needs/problems at appropriate time points.QUALIFICATIONSCurrent and unrestricted Georgia LPN licensureA minimum of one year nursing experience preferredStrong medical surgical, geriatric care experience preferredHome Health experience a plusCurrent CPR certificationGood organizational and communication skillsAvailable to travel within Spalding and surrounding counties.Timely report submissionsCommitment to scheduling and visiting a minimum of 10 clients per monthFor this position, a background check is required.
