This is an entry-level opportunity for individuals with customer-facing experience who want to grow in healthcare coordination and patient services. You will interact directly with patients and healthcare providers, ensuring accurate documentation and timely processing of referrals and applications. Responsibilities for Care Coordinator – Remote Apply job skills and company policies to complete assigned tasks efficiently. Create and complete accurate referrals and applications in a centralized CRM system. Resolve patient questions and concerns regarding their care journey. Enter detailed and accurate information into proprietary software while speaking with patients. Place outbound calls for follow-ups and confirmations as needed. Provide empathetic support while maintaining high-quality documentation. Collaborate with patient referral sources, including healthcare providers. Steward patient accounts from initial contact through final approval or denial. Required Skills & Qualifications Care Coordinator – Remote High School diploma or equivalent. 2–4 years of related customer-facing experience or equivalent combination of education and experience. Strong verbal and written communication skills; professional and empathetic demeanor. Ability to multitask while using software systems and video conferencing tools. Minimum typing speed of 40 WPM; accurate transcription of notes while speaking. Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint). Experience with video conferencing platforms such as Microsoft Teams or Zoom. Ability to work independently in a quiet, distraction-free home environment. Reliable high-speed internet (minimum 15 Mbps download / 5 Mbps upload, hardwired).
