Our expert physicians create cutting-edge treatment plans that best serve the needs of each individual and improve their quality of life.Responsibilities:Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessmentFunctions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patientsOrders and interprets diagnostic and therapeutic tests relative to patient?s age-specific needsAssists with the provision of care in accordance with facility, state, and federal regulationsMonitors the effectiveness of interventionsMaintains relationships with all appropriate contacts in facilitiesEducates family members, clients, and prospective accounts regarding services and programsAssists with the development and maintenance of physician relations, as assignedCompletes weekly, monthly, and other reports, as assigned, in a timely and comprehensive mannerOther Duties as assigned by Medical DirectorQualifications:Current Nurse Practitioner/ Physician Assistant LicenseDegree from an accredited Nurse Practitioner/ Physician Assistant ProgramACLSCurrent infection control certificationStrong clinical collaboration with physiciansProficient computer skillsPACS and EMR experience preferredCommitted to professional growth and learningClean driving recordPersonally owned vehicle requiredLocal travel requiredNot listed on the OIG exclusion listPreferred Experience:Vascular Care: Venous Insufficiency, Peripheral Arterial DiseaseImage Guided InjectablesAdvanced GeriatricSkilled Nursing Facilities, Assisted Living FacilitiesBenefits:401(k)Dental insuranceHealth insurancePaid time offVision insuranceFSA Plan#INDNP
