Community Care Navigator

Job Summary: Responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of healthcare services. Responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family. Performs other duties as assigned. Minimum Requirements: Bachelor's Degree in Social Work or Certified Community Health Worker required. 2+ years of social work or care coordination experience in a healthcare setting, especially using an integrated care model, is preferred.
