We are seeking an Advanced Practice Provider to join one of the nations most integrated healthcare systems, with over 1,300 providers in Indianapolis, Indiana.HighlightsCommunity Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Indianapolis, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.0.6 FTE position, weekdaysThis provider will work autonomously and collaboratively with our palliative care multi-disciplinary teams as a "float" provider at our network hospitals (Community Hospitals North, East, South, Community Heart & Vascular Hospital, Community Howard Regional Hospital, and Community Hospital Anderson) to deliver palliative and health care services to patients.
