The provider will focus on preventive care, patient education, and promoting healthy lifestyles while working within a collaborative care team.Key ResponsibilitiesProvide prenatal, obstetric, gynecological, and well-woman careConduct patient assessments, physical exams, diagnoses, and treatment plansPrescribe medications and coordinate referrals as neededMaintain productivity standards (approx. 2.3-2.6 patients per hour)Provide patient education on preventive care and healthy lifestylesParticipate in quality improvement and community health initiativesMaintain accurate clinical documentation and continuity of care QualificationsGraduate of an accredited Nursing program with advanced trainingActive Illinois license , DEA, and Controlled Substance registrationBilingual English/Spanish requiredStrong communication and patient care skills Schedule40 hours per week (4 clinic days + 1 admin day)1 Saturday per month BenefitsMedical, Dental, Vision Insurance26 Days PTO + 10 Paid HolidaysCompetitive benefits package
