In this dynamic role, you'll provide direct care to patients and support to families, while also serving as a floating clinician-temporarily assigned to different hospice branches based on census needs or staffing gaps. If you're energized by new settings, adaptable to change, and passionate about purpose-driven care, this role could be a perfect fit.As a Hospice RN - Regional Support, You Will:Deliver high-quality, compassionate nursing care for hospice patientsManage and implement individualized plans of careSupport and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)Assess and address patients' physical, emotional, spiritual, and environmental needsAccurately document all care provided and ensure compliance with hospice protocolsEducate families and caregivers on care instructions, medications, and safetyParticipate in the on-call rotation and provide after-hours care as neededCollaborate with physicians, referral sources, and the care teamAssist with referrals, regulatory documentation, and payer complianceTravel to various Gentiva hospice locations as assigned to fill urgent needsSchedule & Travel:This role typically follows a Monday-Friday schedule, with occasional on-call responsibilities.
