Thisindividualwillserveasa guideforthemember andtheirfamiliesto ensurethe member meets withthe rightspecialists,insideoroutsidethe center,inatimelymanner.Anidealcandidatewill haveadeepunderstanding of the healthcare ecosystem,havestrongattentionto detail,andbe drivenbyapassion for providingqualitycareto seniors.Duties/Responsibilities:PartnerwithArchWellHealth'smulti-disciplinarycareteamtogainadeepunderstandingof eachmember's careneeds,includingidentifying gapsand barriersto treatmentCoordinatereferralstoexternalproviders, vendors, and servicestoensurecare needsaremetCompleteproperdocumentationformemberreferralsto ensurerecordsareaccurateandcurrentBuild and nurture relationships with members andtheir families to successfully facilitate care coordinationCarry outall activities andprovide ongoingmonitoringas outlinedin amember'scare plan toensure routine followup and progressionis made on care plan goalsAdheretoHIPAAstandardsandconfidentialityofprotectedhealthinformation(PHI)Promoteclearcommunicationamongcareteamandtreatingclinicianstoensurepositive member outcomesAssist members in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).Be the system navigator and point of contact for members and families, with patients and families having direct access for asking questions and raising concerns.Additionalduties,asassignedRequiredSkills/Abilities:Deepunderstandingofthehealthcareecosystem, ideally with experience in benefits and insurance authorizationPassionforhelping seniormemberssuccessfullycarryoutcareplansAbility towork effectively with diverse populationsincluding staff, providers, members,family members, insurance carriers, vendors, and the publicBasic medical terminologyUnderstanding of Medicare criteria for equipment and suppliesKnowledge of CPT and ICD-10 codesWorkingknowledgeof specialistsandcommunityresourcestoassistthememberStrongcommunication and rapport-building ability amongan interdisciplinary teamAbility to multi-task and manage competing prioritieswith high attention to detailProficient PC skills to include Microsoft OfficeFluency in Spanish, or languages spoken by the people inthecommunitieswe serve (where necessary)Education and Experience:H.S.
