The Job Title will provide direct support to clients in their homes, assisting with daily living activities such as personal care, mobility, meal preparation, and light housekeeping. The role includes monitoring client well-being, documenting observations, and promptly reporting any changes in condition to supervisors or healthcare partners. The Job Title will also offer companionship, engage clients in appropriate activities, and follow individualized care plans to support client goals and safety. Collaboration with family members, colleagues, and other care professionals is essential to ensure consistent, high-quality service.Qualifications Demonstrated ability to provide compassionate, client-centered care, including assistance with personal care, mobility, and daily living activities.Strong communication and interpersonal skills to interact effectively with clients, families, and interdisciplinary care teams.Reliable time management, organization, and attention to detail for following care plans and completing documentation accurately.Capacity to maintain professionalism, confidentiality, and respect for diverse backgrounds in all client interactions.High school diploma or equivalent; relevant certifications (such as CNA, HHA, or similar) are preferred.Previous experience in home care, assisted living, or a related healthcare setting is beneficial.Ability to meet physical requirements of the role, including safe client transfers and extended periods of standing or walking.Willingness to undergo background checks, reference checks, and any required health or licensing clearances.
