Director of Patient Care ServicesHome Sweet HomePosition SummaryHome Sweet Home is seeking an experienced and compassionate Director of Patient Care Services (DPCS) to lead clinical operations and ensure the highest standard of patient care across our home health services. This leadership role is responsible for overseeing clinical staff, maintaining regulatory compliance, driving quality outcomes, and ensuring patients receive safe, coordinated, and exceptional care in the home setting.The ideal candidate is a strong clinical leader with home health expertise, excellent knowledge of Medicare and state regulations, and a passion for mentoring teams while building a culture of accountability and excellence.Key ResponsibilitiesClinical Leadership & OversightProvide day-to-day leadership of all clinical home health operations.Ensure patient care is delivered in accordance with physician orders, agency policies, and applicable regulations.Oversee patient admissions, case assignments, care planning, recertifications, transfers, and discharges.Review and monitor clinical documentation for accuracy, timeliness, and compliance.Serve as the clinical authority and escalation point for patient care concerns.Team ManagementSupervise and support nurses, therapists, aides, and other clinical personnel.Recruit, train, coach, and evaluate clinical team members.Ensure staff competencies, licenses, certifications, and ongoing education remain current.Promote a positive, accountable, and team-focused work environment.Quality & ComplianceMaintain compliance with Medicare Conditions of Participation, licensing standards, and accreditation requirements.Lead QAPI initiatives, chart audits, incident investigations, corrective actions, and survey readiness efforts.Monitor patient outcomes, hospitalization rates, satisfaction scores, and clinical KPIs.Implement performance improvement strategies to strengthen outcomes and efficiency.Operational PartnershipCollaborate with leadership on census growth, staffing models, scheduling, and productivity.Support intake and referral processes to ensure timely start of care.Assist with policy development, workflow improvements, and strategic planning.Participate in on-call leadership support as needed.QualificationsRequiredActive Registered Nurse (RN) license in the state of practice.Minimum 2 years of home health experience.Minimum 2 years of leadership, supervisory, or management experience in healthcare.Strong knowledge of home health regulations, OASIS, and documentation standards.Current CPR certification.Valid driver’s license and reliable transportation.PreferredPrevious experience as a Clinical Manager, DON, or Director in home health.Survey or accreditation experience.Bilingual English/Spanish a plus.Skills & CompetenciesClinical leadershipRegulatory complianceStaff development and coachingOASIS knowledgeQuality improvementStrong communication skillsProblem solving and decision makingCompassionate patient-centered mindsetTime management and organizationPerformance MeasuresSuccess in this role will be measured by:Survey readiness and compliance outcomesStrong patient satisfaction scoresReduced rehospitalization ratesTimely and accurate documentationStaff retention and engagementAchievement of operational and growth goalsEffective leadership and team developmentWork EnvironmentThis role may require a combination of office work, field visits, standing, walking, sitting, computer use, driving, and occasional lifting as needed.
