This role ensures the effective utilization of healthcare resources and monitors care delivery to support optimal clinical and operational outcomes. The Inpatient Care Manager collaborates closely with the Supervisor or Manager of Case Management and leads interdisciplinary efforts to assess, plan, implement, coordinate, monitor, and evaluate services that maximize patient health and ensure safe, timely transitions of care.Principal Accountabilities:* Manage, coordinate, and facilitate patient care through comprehensive patient and family assessments, including social determinants of health, and collaboration with the interdisciplinary care team.* Conduct daily chart reviews, utilization reviews, and coordination of discharge planning activities, ensuring complete, accurate, and timely clinical documentation in the electronic medical record.* Lead and participate in interdisciplinary patient care rounds, identifying and resolving quality of care concerns, barriers to care progression, and discharge delays.* Formulate, coordinate, and implement individualized plans of care and discharge plans to ensure safe, timely, and effective transitions.* Collaborate with physicians, nursing, social work, and other healthcare professionals to manage and support patient care plans.* Perform variance tracking and outcome analysis, identifying trends and opportunities to assess the effectiveness of clinical pathways and care management processes.* Perform other related duties as assigned.
