This is a per diem position in which the candidate will provide their services during the day, Monday through Friday with occasional weekends. The ideal candidate will be responsible for providing care and services of selected member populations across the continuum of illness, ensure effective utilization and monitors health care resources, manage the interdisciplinary team to achieve optimal clinical and resource outcomes, work with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor and evaluate services and outcomes to maximize the health of the patient.Our core values encompass patient-centered care and ensuring our staff are well taken care of. We have exceptional care teams, reasonable schedule expectations, and a robust salary and work/life alignment.Principal Accountabilities:Care Coordination & Patient AdvocacyManage, coordinate, and facilitate care delivery through comprehensive patient and family assessments.Collaborate with multidisciplinary teams to support optimal clinical and psychosocial outcomes.Utilization & Documentation OversightConduct daily chart and utilization reviews to ensure appropriate level of care.Maintain accurate, timely, and comprehensive clinical documentation in the electronic medical record (EMR).Interdisciplinary CollaborationLead and participate in interdisciplinary care rounds.Identify and proactively address quality-of-care concerns, barriers to progression of care, and discharge delays.Discharge PlanningFormulate, coordinate, and implement individualized discharge plans.Ensure safe, timely, and effective transitions across the continuum of care, including post‐acute services and community resources.Resource & Outcome ManagementMonitor healthcare resource utilization and length of stay.Partner with the Supervisor/Manager of Case Management to evaluate outcomes and improve care delivery processes.
