Home Health AideThe Home Health Aide (HHA) at Heritage Home Healthcare provides compassionate, high-quality in-home personal care and support services to clients in the Albuquerque, NM area under the supervision of a Field Supervisor and Registered Nurse. In this role, the HHA assists with Activities of Daily Living (ADLs) and promotes client safety, comfort, independence, and dignity in the home setting while maintaining compliance with NM DOH and CMS regulations.Essential functions include:Provide personal care services including bathing, grooming, oral hygiene, dressing, toileting, and incontinence careAssist with mobility, transfers, ambulation, and positioning using proper body mechanicsAssist with feeding and meal preparation according to dietary guidelines and care plansProvide skin care and support pressure injury preventionMaintain a clean, safe, and organized environment related to client careSupport clients with cognitive impairments, dementia, or behavioral health needs per the Plan of CareObserve, document, and report changes in client condition, behavior, or environment to the supervising nurse or agency staffFollow the Plan of Care and communicate when care needs fall outside scope or instructionsFollow infection control standards and universal precautionsUse personal protective equipment (PPE) appropriatelyMaintain HIPAA compliance, patient rights, and professional boundariesReport suspected abuse, neglect, or exploitation per NM law and agency policyParticipate in required training, competency evaluations, and in-service educationJob qualifications include:High school diploma or equivalentCompletion of a state-approved Home Health Aide training program or CMS-equivalent competencyAbility to pass background check, abuse registry check, and drug screeningValid driver's license and reliable transportationCurrent CPR/First Aid certification or ability to obtainAbility to read, write, and communicate effectively in EnglishPhysical requirements include:Ability to lift, push, pull, and carry up to 50 lbsFrequent standing, walking, bending, and reachingAbility to assist with transfers and mobility supportAbility to work in varied home environmentsAt Heritage Home Healthcare, we are committed to delivering compassionate, personalized care that helps individuals maintain independence and dignity in their homes.
