This role includes providing comprehensive primary care services with an emphasis on geriatric and chronic care management, managing patient health assessments, diagnosing and treating medical conditions, and promoting patient education and preventative care. The practitioner will collaborate with a multidisciplinary team to deliver high-quality care, with the flexibility to perform tasks remotely while maintaining a presence for in-person patient consultations in the patient's home.QualificationsStrong expertise in Nurse Practitioner and Nursing practices, with the ability to assess, diagnose, and provide treatment.Proficiency in Primary Care to manage a wide range of patient needs effectively.Solid understanding of clinical medicine and patient care for diverse populations.Excellent interpersonal, communication, and patient counseling skills.Ability to work both independently in a remote setting and collaboratively within a team in a clinical environment.Active Nurse Practitioner license in Texas and relevant certifications.Experience with electronic medical records (EMR) systems is preferred.A commitment to patient-centered care and community health improvement.
