Guided by our mission to support older adults in living with dignity, purpose, and connection, we provide a comprehensive range of programs and services that meet the evolving needs of the communities we serve.Role overviewAs a Service Coordinator at Village Center and Swartzberg House, you will play a key role in supporting older adults by connecting them with essential services that promote independence, well-being, and quality of life. Working closely with residents, families, housing management, and community providers, you will assess individual needs, coordinate resources, and advocate for appropriate support services. This role combines relationship-building, care coordination, and problem-solving to ensure residents have access to the programs and assistance they need to thrive in a supportive housing environment.This is a hybrid role.What You'll DoEssential responsibilitiesConducts assessments within the first two weeks of a new tenant's residency to determine individuals' ability to function with Activities of Daily Living, and to learn their interests.Arranges for other needed services, i.e., homemaker, transportation, home-delivered meals, and personal care.Conducts at minimum annual reassessments.Documents and reports changes in residents' functioning.Works with residents and, at times, their families in meeting residents' needs.Works with discharge planners and rehabilitation programs for hospitalized residents.Provides residents with information on and referral to available government supported services (Nursing Home Services, Medicare/Medicaid Benefits, State Health Insurance Program, Medicaid Spend-Down Program, Food Stamps, QMB, Circuit Breakers).Works with providers in socialization, education, and health related events.Serves as advocate when needed.Compensation$21.63 per hour; hourly, non-exempt; part-time: up to 24 hours per week.ScheduleMonday to Friday, 8:30 am to 5:00 pm, part-time: up to 24 hours per week.Union statusMembership in Service Employees International Union Local 73 is an option.Who You AreEducationAt least a bachelor's degree in social work, gerontology, or a related field is required.ExperienceAt least 2 years of experience is required.OtherBilingual Russian and English fluency where appropriate.Why This Role RocksImpactIn this role, you will make a meaningful difference in the lives of older adults by helping them access the services and support they need to live safely, independently, and with dignity.
