Position SummaryThe Western Dakota Clinic RN provides professional nursing services by performing patient assessment, triage, care planning, and care coordination within a busy outpatient clinic environment. This role supports providers through rooming, clinical decision-making, patient education, and care management activities for individuals with both acute and chronic conditions. Working as part of a collaborative interdisciplinary team, the Clinic RN contributes to safe, timely, and evidence-based care while helping ensure efficient clinic flow and clear communication. This position fosters a supportive care setting where patients and families feel informed, respected, and actively engaged in their care plans.Key ResponsibilitiesPerform comprehensive nursing assessments, triage patients in person and by phone, and collaborate with providers on care plans.Administer medications, treatments, and immunizations safely and accurately in accordance with provider orders and nursing standards.Provide patient and family education, care coordination, and support for chronic disease management and preventive care.Use the electronic health record to document assessments, interventions, and communications clearly and timely.Participate in population health and outreach activities, including recall lists, care gap closure, and follow up on test results.Support quality improvement, safety initiatives, and clinic flow optimization through active participation in team meetings and projects.
