Home Health Nurse / LPN

Job Summary We are seeking a dedicated and compassionate Home Health Nurse (RN/LPN) to join our team in Indianapolis. In this role, you will deliver personalized care to patients in their homes, ensuring they achieve optimal health and independence. Key Responsibilities o Provide direct nursing care, including wound care, medication administration, and disease management. o Conduct comprehensive assessments of patients’ physical, emotional, and psychosocial needs. o Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams. o Educate patients and caregivers on health management and disease prevention. o Monitor and document patient progress and update care plans as needed. o Coordinate with other healthcare professionals, such as therapists and social workers, to provide holistic care. o Ensure compliance with all state and federal regulations, as well as organizational policies. Qualifications Required: o Current and unencumbered RN or LPN license in Indiana. CPR/BLS certification. o Valid driver’s license and reliable transportation. Preferred: o Previous experience in home health care or related field. o Strong interpersonal and communication skills.
