Job SummaryThe Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care. This role involves conducting assessments, developing care plans, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory guidelines.Key ResponsibilitiesConduct comprehensive patient assessments and ongoing evaluationsDevelop, implement, and manage individualized care plansPerform timely home visits and ensure continuity of careEducate patients and caregivers on disease management and treatment plansMaintain accurate and compliant clinical documentation (DOH, OMIG, etc.)Monitor patient progress and recommend care plan adjustmentsRespond to emergencies and changes in patient conditionSupervise and guide Home Health AidesCoordinate care with physicians, therapists, and healthcare teamsParticipate in quality improvement initiativesRequired QualificationsActive RN License - New YorkBilingual (English/Spanish) requiredMinimum 2-3 years of clinical experienceStrong knowledge of home health practicesValid driver's license and willingness to travel for home visitsPreferred QualificationsExperience in home health or certified home health agenciesFamiliarity with DOH, HRA, MCO, MLTC guidelinesStrong wound care, chronic disease management, and patient education skillsWork EnvironmentField-based role (patient homes)Local travel requiredIndependent work with team collaborationAdditional NotesProof of vaccination required (medical/religious exemptions applicable)Strong focus on serving diverse and underserved populations
