We are excited to offer an excellent opportunity for a Registered Nurse (Home Health) to join a leading healthcare organization in Plano, Texas . This is a 14-week assignment offering a great opportunity to deliver meaningful, patient-centered care while gaining valuable experience in a home health setting.Job Title: Registered Nurse (Home Health)Location: Plano, TX 75074Local Rate: $48 - $52 (Hourly)Schedule: 8-Hour Shifts (40 hrs/week)Start Date: 04/20/2026Key Responsibilities:Assess home health and hospice patients in both routine and emergent situations to identify physical, psychosocial, and environmental needsDevelop, implement, and continuously evaluate individualized Plan of Care (POC) to ensure quality and continuity of careProvide skilled nursing care across diverse patient populations in home settingsPerform comprehensive patient assessments, including pain evaluation and symptom managementAdminister medications and treatments, including IV therapy, venipuncture, and inhalation therapyDeliver wound care, catheter care, and management of drains and durable medical equipmentEducate patients and families on disease management, medications, home safety, and self-care practicesMaintain accurate, timely, and compliant documentation using electronic medical records (Homecare Homebase & OASIS)Coordinate care with interdisciplinary teams, physicians, and referring agenciesSupervise and provide clinical direction to support staff (HHA/STNA/LVN)Ensure compliance with infection control protocols and regulatory standards (OSHA)Monitor cases to meet third-party payer and regulatory requirementsParticipate in on-call rotation and provide support to patients and families as neededAttend interdisciplinary meetings and collaborate to improve patient outcomesServe as a liaison between patients, families, and healthcare teams to ensure effective communicationRequirements:Active State Registered Nurse (RN) LicenseMinimum one (1–2) years of RN experience in a clinical care settingMinimum two (2+) years of home health experience requiredCurrent CPR/BLS certification requiredProficiency with Homecare Homebase software and OASIS documentation (required)Strong clinical skills including IV therapy, venipuncture, wound care, and catheter managementKnowledge of medication administration and pain management techniquesAbility to assess and address physical, emotional, social, and spiritual patient needsExperience in patient and family education and care coordinationAbility to work independently and manage time effectively in a home care environmentPride Global offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts with preferred vendors.
