Pediatric RN Supervisor – Home Health (CHHA)

We are seeking an experienced and compassionate Pediatric Registered Nurse (RN) Supervisor to join our Certified Home Health Agency (CHHA) team. In this leadership role, you will oversee and support nursing staff while ensuring the delivery of high-quality, patient-centered care to pediatric patients in the home setting.The ideal candidate has a strong clinical background in acute pediatric hospital settings, along with leadership or supervisory experience. CHHA experience is a plus.Key ResponsibilitiesClinical Oversight & Patient CareProvide clinical supervision and guidance to field nurses caring for pediatric patientsConduct patient assessments, including admissions and supervisory visitsEnsure individualized plans of care are appropriate, updated, and followedMonitor patient progress and intervene as needed to ensure optimal outcomesCollaborate with physicians and interdisciplinary team membersStaff Supervision & SupportSupervise, mentor, and support RN staff in the fieldConduct performance evaluations and provide ongoing feedbackAssist with orientation and training of new nursing staffServe as a clinical resource for complex pediatric casesCare CoordinationCoordinate care across interdisciplinary teams, including therapists, aides, and physiciansParticipate in case conferences and care planning meetingsEnsure services are delivered according to physician orders and agency standardsDocumentation & ComplianceReview and ensure accuracy of OASIS documentation and clinical recordsMaintain compliance with state, federal, and agency regulationsOversee timely completion of all required documentationOn-Call ResponsibilitiesParticipate in rotating on-call coverageProvide clinical support and guidance during on-call hoursQualificationsActive Registered Nurse (RN) license in New York StateAcute pediatric nursing experience requiredMinimum 2 years of pediatric nursing experience requiredPrior supervisory or leadership experience preferredCHHA experience is a plusStrong knowledge of OASIS and home health regulations preferredExcellent communication, organization, and leadership skillsValid driver's license and ability to travel to patient homesPhysical RequirementsAbility to travel to patient homes, including walk-ups and varied environmentsAbility to stand, walk, bend, and lift as neededAbility to lift up to 50 pounds unassisted and more with assistanceHealth RequirementsPre-employment and annual health assessmentsCompliance with all New York State immunization and health requirementsThis is an excellent opportunity for a pediatric nurse leader with strong acute care experience looking to transition into a leadership role in home health while making a meaningful impact on patient care.Apply today to join a dedicated and growing organization.Job Posted by ApplicantPro
