HC&N Healthcare Solutions - Social Worker Full-Time | Skilled Nursing Facility (SNF) Salary: $30 an hourA well-established Skilled Nursing Facility is seeking a compassionate and driven Social Worker to join our interdisciplinary care team. This role is ideal for someone who is passionate about supporting residents and their families while ensuring a smooth care experience from admission through discharge.Position OverviewThe Social Worker is responsible for addressing the psychosocial needs of residents, coordinating care plans, and facilitating safe and effective transitions. This position plays a key role in enhancing resident quality of life and overall satisfaction.Key ResponsibilitiesEvaluate residents' social, emotional, and psychological needsDevelop and implement individualized care plans in collaboration with the clinical teamParticipate in interdisciplinary care plan meetingsCoordinate admissions, transfers, and discharge planningAssist with arranging home care services, durable medical equipment (DME), and community resourcesProvide counseling and support to residents and their familiesEnsure residents' rights are maintained and advocate for their well-beingAssist with financial documentation and educate residents/families on available resourcesMaintain accurate and timely documentation in accordance with regulatory standardsQualificationsBachelor's Degree in Social Work (BSW) or related field required; MSW preferredPrior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferredStrong communication and interpersonal skillsKnowledge of discharge planning and community resourcesAbility to manage multiple responsibilities in a fast-paced environmentFamiliarity with EMR systems; PCC and MDS 3.0 experience a plusCompensation & BenefitsSalary $30 an hourCompetitive benefits package including:Medical, Dental, and Vision InsurancePaid Time Off (PTO)401(k) Retirement PlanOpportunities for career growth and advancementWe are proud to be an Equal Opportunity Employer.
