Come join our team!The Social Worker, Care Manager is responsible for assessing Care Management members' social, emotional, and environmental needs, developing participants' social plans of care, negotiating of community health services required under the plans of care, and ongoing monitoring and re-assessment of Care Management members. This position will travel to locations within the VIVA HEALTH service area through a reliable means of transportation insured following Company policy.REQUIRED:BSWThree years in social workCurrent LBSW license in good standing in the State of AlabamaValid Driver's license in good standingMay require significant face-to-face member contact with duties regularly performed away from the principal place of businessWilling to submit to vaccine testing and screeningExcellent interview and telephone skills, as well as the ability to deal with older adults in a caring and helpful mannerWorking knowledge of and ability to access community resourcesWorking knowledge of health-related service delivery systemsExcellent communication and relationship skillsAbility to analyze varied environmental factors concerning members' wellbeingOrganization and time management skillsAbility to be flexible, adaptable, and able to work effectively in a variety of settingsBasic computer skillsPREFERRED:MSWOne year of social work experience in the field of community resource coordinationCurrent LMSW license in good standing in the state of AlabamaAbility to utilize Microsoft Word, Outlook, and ExcelEqual Opportunity EmployerThis employer is required to notify all applicants of their rights pursuant to federal employment laws.
