The role involves providing high-quality nursing care to residents, including individuals with Alzheimer's disease and dementia.Key ResponsibilitiesProvide direct nursing care to long-term care residentsMonitor, assess, and document patient conditions and changesSupport individualized care plans for residents with Alzheimer's and dementiaCollaborate with interdisciplinary care teams to ensure optimal outcomesMaintain accurate and timely documentation in the EMR systemFollow facility policies, safety standards, and infection control protocolsAssist with rehabilitation and skilled nursing care needsRequired QualificationsActive New York State RN License (required at time of submission)Current BLS certificationMinimum 1 year of Long-Term Care experience preferredCandidates with recent Med-Surg, Rehab, or Acute Care experience will be consideredStrong clinical judgment, communication, and organizational skillsAbility to manage patient ratios in a long-term care settingFor more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
